The contractors' book has data on licenses contractors in various sectors active around the country. This website has an application for entering the community and construction type, and getting names of contractors.

Complaining to the contractors registrar

The contractors registrar deals with complaints against licensed contractors. This website has information on the process of filing a complaint, and complaint forms downloadable as a word document.

Ministry of Science web site provides application forms for aid to studies, as well as financial report forms for grant winners. The forms are viewable online and downloadable as a word document.

On this webpage every sport organization can check the document completion and perform a follow up after its request for support from the Israeli Ministry of Science, Culture and Sport. The Ministry of Trade & Industry Forms

The Ministry of Industry, Trade and Labor focuses on the promotion of economic growth in Israel. The Ministry is engaged in the encouragement and support of export and international commerce, in order to assist Israeli businesses in enhancing their exports and entering new markets abroad.

Autoform

The Ministry of Trade & Industry enables the public to download and peruse the Ministry's official forms with a special 'Autoform' application. Surfers may access forms connected with the Chief Scientists office, the Investment Center, forms pertaining to various payments, etc.

Assistance for Development Regions Forms

Industries located in development regions are eligible for assistance from the Ministry of Trade & Industry. This site includes applications for assistance, repayment forms, payment vouchers for units in development regions, etc. All forms may be filled in directly on the computer

Chief Scientists Office Forms

The Chief Scientists Office of the Ministry of Trade & Industry enables entrepreneurs and industrialists to download the Office's official forms from the Internet. Investment Center Forms

A new service from the Ministry of Trade & Industry. Branches of industry and plants requiring forms from the Investment Center may peruse, download and return them via this system. The site includes application for approval of Authorized Enterprise status, application for approval of donation of equipment, periodical and final reports, and more. All forms may be filled in directly on the computer.

Overseas Trade Forms - Import

This application provides a variety of import-related forms. These include application for an import permit, application for a special permit for importers, provisional release of merchandise, and more. All forms may be filled in directly on the computer.

Overseas Trade Forms

Any industrialist wishing to receive an export license may download the license application form from this site. Export registration forms may also be downloaded directly in Word format. All forms may be filled in directly on the computer.

The Ministry of Finance (MOF) enables public access to forms from its different departments. The info below contains links to the Ministry of Finance Forms for Customs, VAT, Income Tax, Civil Service Commission, and more.

Income Tax Division Forms

Forms from the Income Tax and Real Estate Taxation Division may be downloaded from this application. The forms include tax adjustment, application for tax credit for a disabled relative, medical certificate for tax credit claim, application for tax refund, etc. All forms may be perused or downloaded as Word or Zip files.

Customs & VAT Division Forms

Official forms from the Customs & VAT Division may be perused and downloaded. These include declaration of personal import of commodities, declaration of personal import of vehicle and declaration of temporary export of accompanying possessions for travelers.

Customs & VAT Division - Dealer's Bank Account Debit Authorization

This application details Customs & VAT Division procedures for transfer of periodical reports for payment and refund applications. The forms may be perused and downloaded from the site.

Capital Market Division: Proceeds & Assets Report 1999

This site details the procedures for filling in proceeds and assets reports for Provident Funds. The report should be completed by the Fund manager and delivered to the Capital Market Division. The form may be downloaded as a Word file or printed direct from the site.

Capital Market Division: Form 15

Registration procedure for .gov.il

The ministry of finance has undertaken, in coordination with the Israeli Internet Organization, to manage the registration policy for the .gov.il suffix. The website carries registration procedures, payment for registration procedures and directions on filling registration forms.

Israel Ministry of Interior (MOI) Forms Online

"No more need to leave the house to acquire Interior Ministry forms. The Ministry's forms website (in Hebrew) provides a wide selection of forms, from birth certificate registration, ID certificates and passports through death certificates.

Passport

Any Israeli national wishing to apply for a new passport or extend an existing one may now download the relevant forms directly from this site. The site also includes forms for declaration of theft or destruction of passports. Applicants must go to a branch of the Interior Ministry in order to receive the new/extended passport or certificate.

Israeli Laissez Passer for Foreign Residents

Foreign citizens resident in Israel either as Permanent Residents or Temporary Residents are eligible for an Israeli Laissez Passer. This site enables access to Interior Ministry forms for residents, both adults and minors, and renewal of validity of existing Laissez Passers. The forms may be printed out directly from the site. Residents must go to a branch of the Interior Ministry in person in order to receive the certificate.

Identity Certificate

All Israeli citizens or residents over age 16 are eligible to receive an Israeli ID certificate. On this site it is possible to download application forms for issue of an ID certificate, forms for alteration/update of information on an existing ID certificate and application forms for attachments to ID certificates. The site also provides forms for declaration of theft or destruction of ID certificates. Applicants must go to a branch of the Interior Ministry in person in order to receive the certificate.

Birth Certificate

All residents may request a copy of a birth certificate from the Interior Ministry either for themselves or on behalf of a first-degree relative. To receive this service, residents must fill in the application form for receipt of certification from the population census. The application forms may be downloaded directly from the site.

Certificate

Israeli nationals requiring any official documents from the Interior Ministry may find what they need on this site. The forms available include application for receipt of birth certificate, certification of name change and death certificate. The application forms may be printed directly from the site.

Death Certificate

Israeli citizens may request a death certificate from the Interior Ministry for a deceased first-degree relative. To receive this service, residents must fill in the application form for receipt of certification from the population census. The application forms may be downloaded directly from the site.

Change of Address

All Israeli citizens are entitled to alter their own address or that of their spouse or minor children on the attachment to the ID certificate. In order to register the change of address, the form provided on this site must be filled in and the previous attachment to the ID certificate added.

Change of Name

Israeli citizens wishing to change their name may now download the relevant forms from this Interior Ministry site. To carry out the required change, the public must go in person to a branch of the Interior Ministry.

Visa

Israeli nationals requiring visas, such as for immigration application, application for authorization of bringing a foreign worker into the country, or extension of a foreign worker's work permit may download the relevant forms directly from this site.

Private/Commercial Visit Entry Visa

A foreign national wishing to visit Israel for private or commercial purposes may download the relevant application forms from this Interior Ministry site. In the case of a commercial visit the host business must complete the form. In the case of a private visit the applicant must specify the reason for his visit.

Application for Entry Visa for Sportsmen/women

Foreign sportsmen/women intending to reside in Israel in order to participate in the activities of an Israeli sports body or organization must complete the Interior Ministry's relevant official application form. The form may be downloaded directly from this site.

Entry Visa for Religious Cleric

A foreign religious cleric residing in Israel in the course of his official duties must receive a special visa from the Interior Ministry. The application forms may be downloaded directly from the site. The form must also be signed by an official of the Ministry of Religion.

Visa for Marriage to Israeli Citizen

A foreign citizen marrying an Israeli national is eligible to receive a visa for residence in Israel from the Interior Ministry. The application form may be downloaded directly from the site.

Entry Visa for Student

Foreign students arriving in the country for study at a recognized Israeli educational institution are eligible for a student visa from the Interior Ministry. The visa application must also be signed by the relevant institution. The form may be downloaded directly from the site.

Application for Registration of Firearms

Israeli nationals wishing to receive a license to possess firearms may download the relevant application form directly from this site. The site also includes forms for renewal of firearms license, provision of license to a shooting instructor and provision of license to a firing range supervisor."
